What this service is

Occupational therapy is a registered health service. Simple Roots Therapy works with children, adolescents and adults on daily function — attention and regulation, motor skills, routines, habits, executive functioning, sensory strategies and the environments in which they play out. Crystal holds general registration with the Occupational Therapy Board of Australia.

What this service is not

This is not psychological therapy, psychiatric care, diagnosis of a mental health condition, or crisis support. If you need urgent help, contact your GP, call Lifeline on 13 11 14, or attend your nearest emergency department. We will tell you if we believe your needs sit outside our scope of practice, and will help you find an appropriate referral.

Coaching and non-clinical offerings

Coaching, mentoring and group programs are offered alongside occupational therapy. Where a service is provided as coaching or mentoring rather than as occupational therapy, this is stated clearly before it begins, so that you know which you are receiving and how it is funded.

Outcomes

Occupational therapy is a collaborative process. Progress depends on many factors, including circumstances and what happens between sessions. We do not guarantee any particular outcome.

Fees and payment

Fees are discussed during the free ten-minute call and confirmed in writing before your first appointment. See the sessions and fees page. Payment is due [at the time of the appointment / within 7 days of invoice]. Occupational therapy provided as appropriate treatment is GST free. If you hold private health extras cover that includes occupational therapy, a receipt with a provider number is issued for you to claim.

NDIS participants who are self-managed or plan-managed are charged in line with the current NDIS Pricing Arrangements, under a separate signed service agreement.

Cancellations

Please give at least [24 / 48] hours notice to change or cancel an appointment. Appointments cancelled with less notice, and missed appointments, are charged in full, because the time cannot be reallocated. This is waived where there is a genuine emergency or genuine illness.

For NDIS participants, the NDIS short-notice cancellation rules apply and take precedence over this clause. We will always let you know before claiming a cancellation.

Records and privacy

We keep clinical records as required by law and store them securely. Your information is handled in accordance with the privacy policy, the Privacy Act 1988 (Cth) and the Health Records and Information Privacy Act 2002 (NSW).

Communication

We respond to enquiries within [2] business days during business hours. Email is not secure and should not be used for clinical or urgent matters.

Telehealth

Telehealth sessions are delivered over a secure video connection. Telehealth is not appropriate for every person or every purpose, and we will discuss with you whether it suits your situation.

Your rights

Nothing in these terms limits your rights under the Australian Consumer Law.

Concerns

If you are unhappy with any part of this service, please tell us — we would rather know. You may also contact AHPRA on 1300 419 495, the NSW Health Care Complaints Commission on 1800 043 159, or, for NDIS supports, the NDIS Quality and Safeguards Commission on 1800 035 544. Raising a concern will not affect the service you receive.
